2) apply CPT on the web https://vis.stonybrook.edu/istart/controllers/start/start.cfm , this is web only, you don't need to see VISA adviser to apply CPT, but it is recommended to be advised, they will review your document and give you comment if needed, advising saves time.

3) wait for 2) hardcopy CPT, it is generally picked up at VISA office unless you request post-delivery.
* waiting may take 2 weeks or longer, but all the processes work fluently this could be completed in couple of days.
